Who is CareLoop for?

CareLoop benefits your residents' families and your staff.

For families, it’s a comprehensive aged care and end-of-life support network. 

It also allows your teams to focus on delivering great clinical and emotional care, while we take care of end-of-life tasks you don’t always have the time or skills to address.

For staff, our 360-Degree Plan also unlocks free grief counselling, to support those who support others.

What support is available to families?

Support is available through every step of the aged care onboarding through to end-of-life process. This includes access to: 

  • A personalised new to aged care onboarding plan (which can be personalised to your facility)
  • tools that help families of residents plan for a death
  • wills and estates lawyers
  • an end-of-life concierge team that can take care of admin tasks

How do families access support?

Families can access the support they need through a personalised online dashboard we create for them. If they prefer to talk to someone on the phone, they can get in touch with our end-of-life concierge team on 03 9968 2111

How involved do our staff need to be?

We’re here to make your job as simple as possible. Once you’ve educated your residents and their families on the benefits of CareLoop, you can leave the rest to us.

We’ll create their personalised dashboard and provide ongoing support for as long as they need. Plus, our end-of-life concierge team is available 24/7 to answer any questions they have.

Can we customise care plans?

Absolutely. Our team can add tasks to your families’ dashboards that are specific to your aged care centre. For example, you might want to include a task to complete certain documents you require, or recurring logistical steps relevant to your facility.

What does it cost?

We offer our Basic Care plan free for all facilities. Facilities can book a chat to discuss personalised pricing for our Standard Care and 360-Degree Care plans.
